Discharge Co-ordination
Sycamore Care will co-ordinate with the team responsible for your discharge from hospital. We'll ensure that you'll be transported safely to your home, and ensure that your home is well prepared for you to arrive and settle back in.
Follow-Up Visits
Once you are settled back in your home with everything that you need, our team can follow-up with additional visits for the duration of your recovery. We recommend that visits continue, so that our friendly staff can ensure your well-being.
